You'll need a minimum of five Rift Keystone Fragments to open the portal. To open up a portal towards one of these sprawling dungeons, simply approach the Nephalem Obelisk located back at camp, and then click on it. These bosses typically take the form of an Elite or boss creature from the main game, albeit a version with a much bigger health pool and even more dangerous damage abilities.
Once you've filled it up completely, a special Nephalem Boss will appear on the battlefield. These multi-level dungeons, randomly crafted out of the environments of the core game, challenge you to clear out enough enemies to fill a bar that records your reign of destruction.
